Hartfalen en COPD

Samenvatting

Er is veel overlap tussen hartfalen en COPD wat betreft diagnostiek, etiologie, prognose en behandeling.

Bij iedereen met COPD dient men ook te denken aan hartfalen. Niet-herkend hartfalen komt bij deze groep patiënten veel voor. Er is ook veel overlap in klachten. Kortademigheid is een kernsymptoom voor beide ziekten. Piepende ademhaling en hoesten komen ook bij hartfalen voor, terwijl verminderde inspanningstolerantie en vermoeidheid ook veelgehoorde symptomen zijn bij patiënten met COPD.

Gemeenschappelijke oorzaken voor COPD en hartfalen zijn roken en systemische inflammatie. COPD en hartfalen zijn chronisch progressieve ziekten die elkaar in negatieve zin versterken. Cardioselectieve bètablokkers mogen wel worden voorgeschreven aan patiënten met COPD.
