Samenvatting
Hoewel inmiddels een indrukwekkend arsenaal aan effectieve antihypertensiva beschikbaar is, bestaat er nog altijd een categorie hypertensiepatiënten die geen aanvaardbare bloeddruk bereikt, ondanks het gebruik van een combinatie van verschillende antihypertensiva. Vaak leidt dit tot een geschiedenis van frequente aanpassingen van medicatie, maar meestal zonder resultaat. Dit kan buitengewoon frustrerend zijn voor arts en patiënt, en leidt niet zelden tot een gelaten mismoedigheid bij beiden ten aanzien van het effect van medicatie. Het wachten lijkt dan te zijn op een complicatie van de slecht geregelde hypertensie. In de huidige tijd van cardiovasculairrisicomanagement richt men de aandacht echter dan maar op andere risicofactoren die misschien wel beïnvloedbaar zijn. De inertie ten aanzien van hypertensie is te betreuren, omdat deze voorbijgaat aan het feit dat hypertensie juist een van de belangrijkste doodsoorzaken is, niet alleen in onze samenleving, maar inmiddels ook in ontwikkelingslanden.1 Is een berustende houding tegenover therapieresistentie dan inderdaad misplaatst? De teneur van dit hoofdstuk is dat het onaanvaardbaar is om therapieresistentie zomaar te accepteren. In het kader daarvan zullen hierna drie aspecten van therapieresistentie de revue passeren. Het eerste betreft de definitie, frequentie en oorzaken van therapieresistentie, waarin zal blijken dat het niet onder controle krijgen van de bloeddruk niet zomaar therapieresistentie genoemd mag worden. Het tweede gedeelte gaat over een van de frequentste somatische oorzaken van therapieresistente hypertensie, primair hyperaldosteronisme, wat in de huisartspraktijk een weinig bekend en onderschat ziektebeeld is. Ten slotte volgt een beschouwing over renale denervatie, een recente niet-medicamenteuze behandeling van therapieresistente hypertensie die, terecht of onterecht, een hoge vlucht begint te nemen.
