Samenvatting
Geconfronteerd met een ontroostbaar huilend en/of overmatig regurgiterend kind moet de clinicus enkele uitdagingen tegelijk aangaan. Allereerst is er de noodzaak om op een snelle, voor het kind niet (te) belastende manier een goede diagnose te stellen. Dan moeten de vaak ongeruste ouders gecounseld worden. En last but not least moeten, zoals blijkt uit de jongste literatuur, oude gewoontes en aannames bijgesteld worden – en niet alleen die van de ouders. Want een huilend en/of spugend kind blijkt veel vaker niet dan wel een (peptische) oesofagitis te hebben en een aantal van de traditioneel aangevraagde onderzoeken en toegepaste behandelingsstrategieën doorstaan de evidence-based toets niet. De recent gepubliceerde raadgevingen en richtlijnen lezen dan ook even goed als een lijst van ‘don’ts’ als van ‘do’s’. Er is bovendien (veel) meer onderzoek nodig, want het aantal onzekerheden, controverses en vragen is nog groot.
