Zusammenfassung
Placebo- und Noceboeffekte haben über individuelle Erwartungen und Vorerfahrungen von Patient:innen einen direkten Einfluss auf die Wirksamkeit und Verträglichkeit von Behandlungen. Erwartungseffekte in der Krebsbehandlung wurden in Studien für Symptome wie chronische Erschöpfung, Schmerzen oder Übelkeit und Erbrechen nachgewiesen. Erwartungen werden maßgeblich in den Interaktionen mit Behandelnden und über die Patient:inneninformation generiert. Die informierte Aufklärung eignet sich besonders als Ansatzpunkt, um realistische und positive Erwartungen an die Behandlung zu erarbeiten. Hierbei kommen Strategien wie das „framing“, die Aufklärung über Placebo- und Noceboeffekte und die Erarbeitung von Copingstrategien zum Umgang mit Nebenwirkungen zum Einsatz. Wenn es gelingt, Erwartungen angepasst an individuelle Vorerfahren zu optimieren, können positive Behandlungseffekte über den Placeboeffekt zusätzlich verstärkt werden. Auf der anderen Seite lassen sich Noceboeffekte minimieren, um die Belastung durch Nebenwirkungen zu verringern und die Behandlungsadhärenz zu steigern.
Abstract
Placebo and nocebo effects influence treatment efficacy and safety through individual pre-experiences and expectations. Expectation effects in oncology have been shown for symptoms such as fatigue, pain, nausea, or vomiting/emesis. Expectations are generated in patient–practitioner interactions and during provision of patient information. During informed consent, expectation-management strategies can be used to generate realistic yet positive expectations about oncologic treatments. These strategies include information framing, explaining placebo and nocebo effects, and developing individual coping strategies for adverse events. If expectation-management strategies are successfully adapted to individual pre-experiences, positive treatment effects can be additionally enhanced through the placebo effect. At the same time, nocebo effects can be minimized to reduce side effects and increase treatment adherence.
