Zusammenfassung

Die Frage nach geriatrischem Handlungsbedarf entscheidet sich nicht am numerischen, sondern am biologischen Alter des Patienten und am Vorliegen geriatrischer Syndrome. Geriatrische Syndrome sind typischerweise multikausal bedingt und verlangen nach einem multiprofessionellen Diagnostik- und Therapieansatz. Dafür stehen in der Geriatrie etablierte und validierte Instrumente des sog. multidimensionalen geriatrischen Assessments zur Verfügung. Damit lassen sich zunächst funktionelle, kognitive, emotionale und psychosoziale Defizite und Ressourcen eines Patienten erfassen. Basierend auf den Ergebnissen kann dann ein individuell zugeschnittener Therapieplan erarbeitet werden, den ein multiprofessionelles Behandlungsteam umsetzt. Dabei findet das geriatrische Assessment sowohl zur Diagnostik als auch zur therapeutischen Verlaufskontrolle Anwendung. Dieser Beitrag stellt etablierte Assessmentinstrumente vor und erläutert deren Einsatz zur Erfassung geriatrischer Syndrome anhand eines Fallbeispiels.

Abstract

The decision regarding a need for action in geriatric patients is based not on numerical age, but rather on the patient’s biologic age and the presence of geriatric syndromes. Geriatric syndromes are typically multicausal and require a multiprofessional diagnostic and therapeutic approach. To achieve this, established and validated instruments of the so-called comprehensive geriatric assessment (CGA) are available to geriatricians. These enable the patient’s functional, cognitive, emotional, and psychosocial deficits and resources to be assessed. Based on these CGA results, an individually tailored treatment plan can be developed and realized by the multiprofessional therapeutic team. The results of the CGA thus serve diagnostic as well as treatment monitoring purposes. The present article describes established CGA instruments and explains their application for identification of geriatric syndromes based on a case example.
