Zusammenfassung
Die onkologische Rehabilitation älterer Patienten stellt Behandler in jedem Schritt – von der Anmeldung über die Planung bis zur Durchführung – vor besondere Herausforderungen. Neben den typischen tumor- und therapiebedingten Einschränkungen leiden ältere Patienten zunehmend unter Multimorbidität und sind von Gebrechlichkeit bedroht. Körperliche Einschränkungen wie reduzierte körperliche Aktivität, muskuläre Schwäche/Sarkopenie und therapieassoziierte Neuropathien, einhergehend mit Gang- und Standunsicherheiten, sowie neurokognitive Einschränkungen gefährden die Autonomie und die Alltagsaktivität der Patienten. Hieraus ergibt sich ein erheblicher Rehabilitationsbedarf, dem entsprechende Fachkliniken für onkologische Rehabilitation mit spezifischen, individuell ausgerichteten Therapiekonzepten begegnen. Entscheidend für den Zugang hierzu ist jedoch, dass bereits im Akutkrankenhaus der Weg zu einer onkologischen Rehabilitation gebahnt wird. Verschiedene diagnostische Tools und Assessments helfen in der Rehabilitationsdiagnostik bei der Erfassung der individuellen Leistungsfähigkeit der Patienten. Nach Vereinbarung der individuellen Rehabilitationsziele werden dann geeignete sport- und physiotherapeutische, ergo- und ernährungstherapeutische sowie psychoedukative Therapien und Anwendungen verordnet.
Abstract
The oncological rehabilitation of older patients presents challenges for medical practitioners at every step of the process from the registration, to the planning and implementation. In addition to the typical tumor-related and treatment-related restrictions, older patients increasingly suffer from multimorbidity and are threatened by frailty. Physical limitations, such as reduced physical activity, muscular weakness/sarcopenia and therapy-associated neuropathies, accompanied by unsteady gait and standing as well as neurocognitive limitations, endanger the autonomy and activities of daily living of patients. This results in a considerable need for rehabilitation, which specialized clinics for oncological rehabilitation can provide with specific, individually tailored therapeutic concepts; however, for access to this it is crucial that the oncological rehabilitation is already initiated in the acute care hospital. Various diagnostic tools and assessments help in rehabilitation diagnostics to evaluate the individual performance of patients. After the individual rehabilitation goals have been set, suitable exercises, physiotherapy, ergotherapy and nutritional therapy as well as psychoeducational therapies and applications are prescribed.
