Zusammenfassung
Die Einführung der medikamentösen Immuntherapie hat die Behandlung von Tumorerkrankungen in vielen medizinischen Fachgebieten revolutioniert. Trotzdem ist das Verständnis der Regulation des Immunsystems und des Zusammenhangs zwischen Immunsystem, Entzündung und Malignität unvollständig. Ziel dieser Arbeit war, die Rolle des Immunsystems bei malignen und entzündlichen Erkrankungen in der Mund‑, Kiefer- und Gesichtschirurgie besser zu verstehen, um die Grundlage für künftige Therapien zu schaffen. Das Gleichgewicht zwischen Immunität und Immuntoleranz ist bei Entzündungen und Malignomen gestört. Bei Entzündungen zeigt sich eine Verschiebung zu gesteigerter, zerstörerischer Immunität, während bei Malignomen die Immuntoleranz pathologisch gesteigert ist und so ermöglicht, dass sich der Tumor der Abwehrreaktion des Körpers entzieht. Auch chirurgische Maßnahmen beeinflussen die Reaktion des Immunsystems. Die Behandlung entzündlicher und maligner Erkrankungen kann möglicherweise verbessert werden, wenn die chirurgische Behandlung und eine medikamentöse Immuntherapie geschickt kombiniert werden.
Abstract
The introduction of pharmacological immunotherapy has revolutionized the treatment of tumor diseases in many medical specialties. Nevertheless, the understanding of the regulation of the immune system and the relationship between the immune system, inflammation and malignancy is incomplete. The goal of this work was to better understand the role of the immune system in malignant and inflammatory diseases in oral and maxillofacial surgery to lay the foundation for future therapies. The balance between immunity and immune tolerance is disturbed in inflammation and in malignancies. In inflammation, there is a shift towards increased destructive immunity while in malignancy immune tolerance is pathologically increased, allowing the tumor to evade the body’s defense response. Surgical procedures also influence the immune system response. The treatment of inflammatory and malignant diseases can potentially be improved if surgical treatment and pharmacological immunotherapy are skillfully combined.
