Zusammenfassung
Die Korrektur schwerer Fehlbisse ist eine interdisziplinäre Aufgabe von Kieferorthopäden und MKG-Chirurgen. Ziel ist es, die Verzahnung dreidimensional zu normalisieren und dauerhaft zu stabilisieren. Besonders schwere Fehlbisse mit dreidimensionaler Ausprägung werden heute oft zweizeitig im Abstand von 12 Monaten korrigiert. Bezieht man den gesamten Gesichtsschädel und die umgebenden Weichgewebe in die Planung mit ein, ergeben sich neue ästhetische Blickwinkel, die oft zu umfangreichen komplexen skeletalen Verlagerungen des Mittel- und Untergesichts unter Beachtung der Weichgewebekontur und -funktion in allen drei Dimensionen führen. Voraussetzung für ästhetische Operationsergebnisse sind zum einen umfangreiche Planungen des chirurgischen Vorgehens und zum anderen die exakte intraoperative Realisierung. Die Bisskorrektur ist selbstverständlich geworden und heute nur noch ein Teil der umfassenden Konstruktion von Mittel- und Untergesicht. Der vorliegende Artikel beschreibt alle Aspekte der Ästhetik der orthognathen Chirurgie.
Abstract
The correction of severe malocclusions is an interdisciplinary task for orthodontic and maxillofacial surgeons. The aim is a three-dimensional normalization of the dentition and a permanent stabilization. Particularly severe malocclusions with three-dimensional characteristics are currently often corrected in a 2-sided manner after a 12-month interval. If the complete facial skeleton and the surrounding soft tissues are also included in the planning, new aesthetic aspects must be considered which often lead to substantial complex skeletal displacement of the middle and lower facial bones bearing the soft tissue contour and function in all three dimensions in mind. Prerequisites for aesthetic operative results are extensive planning of the surgical approach and exact operative realization. Correction of malocclusions has become a routine procedure and is nowadays only a part of the comprehensive construction of the middle and lower facial structures. The present article describes all aspects of the aesthetics of orthognathic surgery.
