Zusammenfassung
Schleimhautmelanome im Bereich der Mundhöhle stellen eine seltene Erkrankung dar. Anders als bei kutanen Manifestationen ist die Ätiologie nach wie vor ungeklärt. Die hauptsächlich betroffenen Stellen liegen am Gaumen und Oberkieferalveolarkamm. Lange Symptomlosigkeit ist sehr oft der Grund für eine späte Diagnosestellung und die dadurch infauste Prognose. Wir berichten über einen 78-jährigen Patienten, bei dem sich auf dem Boden eines blauen Naevus am harten Gaumen ein Melanom entwickelte. Das Schleimhautmelanom des oberen Aerodigestivtraktes wurde kürzlich als neue Entität in die TNM-Klassifikation der UICC aufgenommen.
Abstract
Primary malignant melanoma of the oral cavity is an exceedingly rare event. The most frequently affected sites are the palate and the gingiva of the maxilla. The lack of symptoms often causes diagnostic delay. We report on a 78-year-old man with a history of melanotic pigmentation on the hard palate for many years. Due to tumor growth an incisional biopsy was performed which yielded the diagnosis of mucosal melanoma arising in a preexisting blue nevus. Mucosal melanomas of the upper airway are not classified as other melanomas but with their own TNM classification due to their special behavior. Although the classification by Clark cannot be applied, thickness and size of the tumor are known prognostic indicators.
