Zusammenfassung
Der AV-Block II. Grades Typ Mobitz ist im EKG durch einen intermittierenden Ausfall der Kammergruppen bei konstant bleibender PQ-Zeit charakterisiert. Durch den diffusen Befall des His-Purkinje-Systems und Progression zum AV-Block III. Grades ist die Prognose ohne Schrittmacherimplantation ungünstig. Der fortgeschrittene AV-Block II mit 2:1-, 3:1-Überleitung kann sowohl im AV-Knoten als auch im His-Purkinje-System lokalisiert sein. Die Lokalisation der Leitungsstörung ist maßgeblich für die Einschätzung der Prognose. Sie kann in den meisten Fällen nichtinvasiv erfolgen.
Abstract
Mobitz II second-degree AV block can be recognized electrocardiographically by an intermittent drop of the QRS complex with unaltered AV conduction time. It is explained by extensive und diffuse damage to the His-Purkinje system. This accounts for the serious prognosis in non-paced patients. Advanced second-degree AV block (2:1, 3:1 block) may be confined either to the AV node or to the His-Purkinje system. The conduction disturbance, which determines the prognosis, can usually be localized using noninvasive measures.
