Zusammenfassung
Breitkomplextachykardien führen häufig zu Schwierigkeiten bei der Differenzierung zwischen Tachykardien mit ventrikulärem und supraventrikulärem Ursprung. Die häufigste Ursache einer Breitkomplextachykardie stellt die ventrikuläre Tachykardie dar, gefolgt von Tachykardien mit supraventrikulärem Ursprung und Aberranz oder Leitung über eine antegrade akzessorische Leitungsbahn. Die Kenntnis evtl. kardialer Grunderkrankungen kann bei der Differenzierung ebenso hilfreich sein wie die Zuhilfenahme definierter EKG-Algorithmen. In der vorliegenden Kasuistik berichten wir über einen 51-jährigen ICD-Patienten mit Breitkomplextachykardien und stattgehabtem Myokardinfarkt in der Vorgeschichte und anamnestisch bestehendem paroxysmalem Herzrasen seit der Kindheit. In der elektrophysiologischen Untersuchung zeigte sich schließlich eine orthodrome atrioventrikuläre Reentry-Tachykardie mit intermittierendem Linksschenkelblock bei links posterior (lateral) gelegener akzessorischer Leitungsbahn, die mittels Hochfrequenzablation erfolgreich behandelt werden konnte.
Abstract
Differentiating between tachycardias of ventricular or supraventricular origin can be challenging when diagnosing wide QRS-complex tachycardias. Mechanisms of wide QRS-tachycardia consist of ventricular tachycardia and supraventricular tachycardias with aberrant conduction or antidromic atrioventricular reentrant tachycardia using an antegrade accessory pathway. Specific algorithms using 12-lead ECG criteria and the existence or absence of structural heart diseases can help distinguish between a ventricular and supraventricular origin. Here, we report on a 51-year-old patient with ischemic heart disease and an implantable cardioverter defibrillator (ICD) suffering from wide QRS-complex tachycardia and recurrent palpitations since childhood. An electrophysiological examination revealed an orthodromic reentrant tachycardia with intermittent left bundle brunch block using a left posterior (lateral) accessory pathway, which was successfully treated using radiofrequency ablation.
