Zusammenfassung

Die Stentangioplastie der A. carotis (CAS) mit zerebraler Protektion hat sich zu einer echten Alternative zur Karotisthrombendarteriektomie (CEA) entwickelt. Bei chirurgischen Hochrisikopatienten ist sie mittlerweile als Behandlungsmethode der 1. Wahl akzeptiert. Ob sie sich zu einer wissenschaftlich fundierten, evidenzbasierten Alternative zur Operation bei Patienten mit normalem Operationsrisiko und mit asymptomatischen Stenosen entwickeln wird, muss vom Ergebnis derzeit laufender randomisierter Studien, wie z. B. CREST, ICSS, ACT I, ACST-2 oder TACIT, abhängig gemacht werden. In der vorliegenden Arbeit wird die Technik der Karotisstentangioplastie erläutert und ein Überblick über die aktuellen Daten aus den randomisierten Studien gegeben.

Abstract

Carotid artery stenting (CAS) with cerebral protection has become an effective alternative to carotid endarterectomy (CEA). In high-risk surgical patients, CAS is now the method of first choice. However, it cannot be considered a scientifically sound and evidence-based alternative to carotid surgery in low-risk patients or in patients with asymptomatic stenoses until the results of trials such as CREST, ICSS, ACT I, ACST-2 and TACIT are available. This overview presents the technique of carotid artery stenting and the currently available data from randomized trials.
