La mucocèle appendiculaire (MA) est une lésion rare, caractérisée par l’accumulation de mucus dans la lumière appendiculaire. Sa localisation sur un appendice de localisation atypique en sous-hépatique est inhabituelle. Nous rapportons un cas observé chez un patient de 56 ans, chez qui le diagnostic préopératoire évoqué était celui d’un léiomyome extramuqueux de la paroi duodénale, orienté par les données de l’imagerie. L’exploration chirurgicale découvrait une volumineuse MA de siège sous-hépatique sur cæcum court, comprimant la paroi duodénale à hauteur du deuxième duodénum qui était indemne. Une résection iléocæcale était réalisée. Les suites opératoires étaient simples. L’histologie avait confirmé le diagnostic de tumeur mucosécrétante bénigne de l’appendice. Les volumineuses MA, développées sur appendice vermiculaire de topographie atypique, sont rarement observées. Leurs aspects radiocliniques sont déroutants. Le diagnostic n’est souvent posé qu’en peropératoire devant une masse kystique développée aux dépens de l’appendice. Le traitement est chirurgical, et l’exérèse doit être complète. Le pronostic dépend de l’histologie.
Abstract
Mucocele of the appendix is a rare lesion, characterized by an accumulation of mucus. Localization of appendicular mucocele under the liver is uncommon. We report a case of a 56-year-old man, with duodenal tumor revealed on abdominal ultrasonography and computed tomography. The preoperative diagnosis showed duodenal leiomyoma. Surgical exploration revealed a voluminous appendicular mucocele, which comprised the duodenal wall, because it was amucocele of an appendix under the liver. Ileocecal resection was performed with uneventful postoperative course after 30 months. Histopathologic examination showed a benign cystadenoma. Clinical and radiological appearance of this rare lesion could be unusual preoperatively and are often confusing. We discuss the diagnostic and therapeutic problems raised by this lesion. Surgical treatment requires a complete resection. Prognosis depends on histological study.
