Le but de notre étude était de décrire la pratique de la ponction-biopsie hépatique transpariétale en ambulatoire et de rapporter ses complications.

Patients et méthodes

Il s’agit d’une étude de pratique clinique qui s’est déroulée du 16 août 2005 au 7 juin 2008 dans le service d’hépatogastroentérologie du centre hospitalier universitaire Yalgado-Ouédraogo de Ouagadougou. Ont été inclus les patients âgés de plus de 18 ans et qui disposaient d’une échographie du foie. Ont été exclus de l’étude les femmes enceintes et tous les patients présentant des contre-indications à la biopsie hépatique par voie transpariétale (troubles de la crase sanguine). Les informations suivantes étaient collectées: l’âge, le sexe, l’indication, le type d’aiguille à biopsie, la taille du fragment biopsique, les complications et le traitement.

Résultats

Au total, 106 biopsies ont été réalisées. Il y avait 77 hommes (72,6 %) et 29 femmes (27,3 %). L’âge moyen était de 39 ans. La longueur moyenne des fragments biopsiques était de 23 ± 6 mm. L’hépatite virale B chronique était l’indication la plus fréquente (75,4 %). Les complications mineures étaient représentées par la douleur (60,5 %) et un malaise vagal (3,9 %). Un cas de complication majeure à type de perforation de la vésicule biliaire a été observé (0,3 %).

Conclusion

Notre étude a montré que la ponction-biopsie hépatique en ambulatoire, sous réserve de certaines précautions, est de pratique sûre, avec un bon degré de satisfaction parmi nos patients.

Abstract

Aim

The aim of this study was to describe outpatient percutaneous liver biopsy and its complications.

Patients and methods

Between August 16, 2005 and June 7, 2008, 106 liver biopsies were performed in the Hepatogastroenterology Department of Yalgado-Ouédraogo Teaching Hospital in Ouagadougou. We included all patients aged over 18 who had had a liver ultrasound. We excluded pregnant women and all patients for whom percutaneous liver biopsy was not to be recommended (blood disorders). Details concerning demographic data, indications, needle biopsy type, length of tissue core, complications and treatment were collected.

Results

A total of 106 liver biopsies were performed. There were 77 male (72.6%) and 29 female (27.3%) patients. The median age was 39 years. The mean length of the tissue core was 23 ± 6 mm. The most common indication for liver biopsy was hepatitis B (75.4%). Minor complications occurred: pain (60.5%), vagal malaise (3.9%). One major complication was seen: gallbladder perforation (0.3%).

Conclusion

Our study has shown that day case liver biopsy in selected patients is safe and associated with a high degree of satisfaction among patients. We are working towards future, non-invasive methods for assessing liver fibrosis in addition to liver biopsy.
