Thoracoscopic Blebectomy and Pleurodesis for Primary Spontaneous Pneumothorax

Primary spontaneous pneumothorax (PSP) may be caused by blebs that can be excised via a minimally invasive approach of thoracoscopy. Pleurodesis may be added to this procedure to reduce recurrence.

The authors of the article entitled “Thoracoscopic Management of Blebs: Resection with/out Primary Pleurodesis” published in this issue of the Journal have described a systematic review of the literature on the topic comparing its outcome with and without pleurodesis [1]. However, a formal definition of blebs is missing as inclusion criteria. Other limitations include non inclusion of additional procedures that may be added through the thoracoscopic approach and need for description of various other methods of pleurodesis available besides talc. It is not a true comparison as pleurodesis was done in 70% cases. Moreover, the pleurodesis was mechanical in majority and chemical in a few. All cases with recurrence had undergone pleurodesis so it is difficult to prove its superiority.
