Résumé
Les causes des exacerbations des broncho-pneumopathies chroniques obstructives (BPCO) sont multiples, infectieuses dans plus de la moitié des cas. Ces exacerbations sont traitées de façon abusive par les antibiotiques alors que seulement 40 % sont d’origine bactérienne. La procalcitonine (PCT) figure actuellement parmi les meilleurs marqueurs biologiques des infections d’origine bactérienne. Nous proposons de montrer l’apport de son dosage dans la réduction de l’antibiothérapie lors des exacerbations de BPCO (EABPCO). Notre population était constituée de 91 patients répar tis aléatoirement en 2 groupes: 50 patients dans le groupe témoin et 41 patients dans le groupe PCT. L’antibiothérapie était guidée dans le groupe PCT par les valeurs de celle-ci (au dessus de 0,25 ng/ml, mise en place d’une antibiothérapie; en dessous de 0,25 ng/ml, pas de traitement antibiotique). Le groupe témoin bénéficiait de la prise en charge usuelle (antibiothérapie délivrée par le médecin traitant selon l’état du malade et indépendamment des valeurs de la PCT). Dans le groupe PCT, 6 malades (14,6 %) ont reçu une antibiothérapie. Dans le groupe témoin, 32 malades (64 %) ont reçu une antibiothérapie, dont seulement 22 % étaient justifiées (11 malades) [PCT ≥ 0,25 ng/ml]. 21 malades (42 %) ont reçu une antibiothérapie en excès, malgré une faible concentration de PCT (< 0,25 ng/ml) [p < 0,01]. Donc la PCT permet de réduire l’usage d’antibiotiques dans 42 % des cas pour lesquels ils auraient été injustifiés. Ainsi Cette étude souligne l’importance de la PCT, marqueur spécifique d’infection bactérienne, pour guider la prescription d’antibiotiques. La PCT devrait donc être intégrée dans des algorithmes décisionnels pour la prescription d’antibiotiques au cours des pathologies infectieuses, notamment lors des EABPCO.
Abstract
The causes of the aggravations of the exacerbation of chronic obstructive pulmonary disease (ECOPD) are multiple, infectious in more than half of the cases. These exacerbations are treated in an abusive way with antibiotics although 40% only have a bacterial origin. Presently, the procalcitonin (PCT) exists among the best biological markers of the bacterial-caused infections. We suggest showing the contribution of its dosage in the reduction of the antibithérapy during the ECOPD. Our population was made of 91 patients, divided in 2 groups: 50 patients in the test - group and 41 patients in the PCT group. The antibiotherapy was guided in the PCT group by the values of this one (over 0.25 ng/ml; establishment of an antibiotherapy, below 0.25 ng/ml no antibiotic treatment). The test group benefited from the usual taking in charge antibiotherapy delivered by the treating doctor according to the patient’s state and independently of the PCT values. In the PCT group 6 patients (14.6%) have received an antibiothérapy. In the test group 32 patients (64%) have received an antibiotherapy. among whom only 22% were justified (11 patients) [PCT ≥ 0.25 ng/ml]. 21 patients (42%) have received an antibiotherapy in excess despite low concentration of PCT (< 0.25 ng/ml) [p < 0.01].Thus, the PCT allows to reduce the use of antibiotics in 42% of the cases for which they would have been unjustified. Then this study outlines, the importance of the PCT, specific marker of the bacterial infection to guide the antibiotics prescription. Hence, the PCT should be, integrated in the decisional algorithms for the prescription of antibiotics in the course of the infections pathologies, notably during The ECOPD.
