Résumé
L’anémie est fréquente chez le sujet âgé de plus de 75 ans. Les mécanismes physio-pathologiques de l’anémie sont identiques à ceux du sujet plus jeune et l’enquête étiologique repose sur des arbres diagnostiques précis. Des facteurs de risque ont été identifiés dans cette tranche d’âge: institutionnalisation, faible niveau socio-économique, comorbidités. Les particularités du sujet âgé tiennent en plusieurs points:

Plus que le taux d’hémoglobine, c’est le retentissement clinique de l’anémie et sa tolérance qui sont à prendre en compte. Cette particularité conduit souvent le gériatre, pour des taux d’hémoglobine peu abaissés, à réaliser un bilan étiologique d’anémie qui ne doit pas exclure, si nécessaire, un myélogramme.

L’anémie est souvent multifactorielle, liée notamment aux différentes comorbidités et/ou aux pathologies intercurrentes aiguës avec pour conséquence dénutrition, inflammation… L’insuffisance rénale chronique et les dysthyroïdies sont également fréquentes.

Il existe de nombreuses anémies carentielles (fer, folates et/ou vitamine B12), dont l’origine doit être identifiée.

Les syndromes myélodysplasiques, hémopathies clonales, ont une prévalence probablement très sous-estimée dans cette tranche d’âge.
De l’enquête étiologique découle la prise en charge du patient, notamment thérapeutique. Pour les SMD de bas grade, les essais thérapeutiques récents avec différentes érythropoïétines sont très encourageants.
Abstract
Anaemia is common in patients older than 75 years. Patho-physiologic mechanisms of anaemia are similar to that involved in younger patients even though certain mechanisms are more common in the elderly. Practical decisional algorithms have to be used for the diagnosis of anaemia. Risk factors for anaemia have been identified in this age group: institutionalization, bad socio-economic conditions, co-morbid conditions. Particular emphasis is given to several points:

The clinical tolerance of anaemia is the most important feature to take into account in older patients, rather than the decline of the haemoglobin level. Even if the haemoglobin level is only slightly decreased, evaluation of anaemia should be performed by the geriatrician, including, if needed, the achievement of a bone-marrow aspiration.

Causes of anaemia are often multiple, due to frequent comorbid conditions and acute illnesses, associated with nutritional deficiencies, inflammation… Chronic kidney disease and dysthyroidia are also common.

Nutritional deficiencies (iron, folate and/or B12 deficiency) are frequent and their cause has to be identified.

The prevalence of myelodysplastic syndromes, which are clonal diseases, is probably underestimated in this age group.
