Zusammenfassung
Eine Studie (Gillin et al. Arch. of General Psychiatry 51: 189–197, 1994) legt einen Zusammenhang zwischen REM-Schlafparametern (“REM-Druck”) und einem späteren Rückfall bei alkoholabhängigen Patienten nahe. Dazu wurden Schlafableitungen ca. 16 Tage nach dem akuten Entzug durchgeführt. In die Replikationsstudie wurden 52 alkoholabhängige Männer eingeschlossen, die keine sekundäre psychische Störung wie Depression und/oder Schlafstörung wie Schlaf-Apnoe oder Periodische Beinbewegungen im Schlaf aufwiesen. Die Ergebnisse konnten die ursprünglichen Befunde nicht replizieren, jedoch zeigte eine umfassende Analyse, daß Schlafvariablen wie Einschlaflatenz und REM-Latenz (tendenziell) neben Sozialstatus und Depressivität ein eigenständiger prädiktiver Wert zukommt. Es scheint sinnvoll, den Zusammenhängen zwischen REM-Schlaf, Schlafstörungen, Depressivität und Rückfall in dieser Patientengruppe genauer nachzugehen.
Summary
Gillin et al. (Arch. of General Psychiatry 51: 189–197, 1994) have shown a relationship between REM sleep parameters (“REM pressure”) and subsequent relapse in patients with primary alcoholism. Therefore, standard polysomnographic recordings were performed 16 days after acute withdrawal. The present replication study included 52 patients with primary alcoholism. Patients with secondary mental disorders such as depression or anxiety disorders and/or sleep apnea syndrome or periodic leg movement during sleep were excluded. The present results did not replicate the earlier findings, however, a comprehensive analysis revealed that sleep variables such as sleep latency and REM latency (only a tendency) have predictive importance in addition to marital status and depressive moods. It seems fruitful to examine the relationships between REM sleep, sleep disorders, depressive moods and relapse in this patient group in a more detailed way.
