Zusammenfassung
Patienten mit einer schlafbezogenen Atmungsstörung (SBAS) bedürfen wegen der häufig gleichzeitig auftretenden Begleiterkrankungen Hypertonie, Herzrhythmusstörungen, zerebrovaskulären Insulten, exzessiver Tagesschläfrigkeit und Leistungsfähigkeitsminderung einer raschen Diagnostik und Therapie. Die nach wie vor limitierte Anzahl an Polysomnographieplätzen im deutschsprachigen Raum als auch die relativ hohen Kosten dieser Untersuchung machen eine kostengünstige und zugleich verläßliche präklinische Diagnostik erforderlich. Das APNOESCREEN I® (AS) ist ein Gerät zum Screening von Patienten, bei denen eine SBAS vermutet wird. In der vorliegenden Studie wurde bei 30 Patienten mit Verdacht auf eine SBAS eine Validierung des AS im Vergleich zu einer standardisierten Polysomnographie (PSG) durchgeführt. Einem Apnoeindex (AI) von 15,8±21,8, einem Hypopnoeindex (HI) von 11,8±11,3 und einem Apnoe-Hypopnoeindex (AHI) von 27,6±27,1 inder PSG entsprachen 25,4±25,2 Atempausen/h geschätzter Schlafzeit im AS. Bei sehr guter Korrelation des Atempausenindex des AS mit dem AI (r=0,96; p<-0,01) und dem AHI (r=0,97; p<-0,01) ergab sich für einen Cut-off-Wert zur Identifizierung einer SBAS von AHI >10 eine Sensitivität von 94% und eine Spezifität von 92%. Für einen Grenzwert von AHI >20 beträgt die Sensitivität 94% und die Spezifität 100%. Unter Berücksichtigung des klinischen Beschwerdebildes des SBAS-Patienten ist das AS damit ein gutes Screeninggerät zur Beurteilung der Notwendigkeit einer weiteren Diagnostik und Therapie bei Patienten mit dem Verdacht auf eine SBAS.
Summary
Sleep related breathing disorders (SRBDs) are associated with a variety of comorbidity such as hypertension, cardiac arrhythmias, cerebrovascular disease, excessive daytime sleepiness and neuropsychologic performance deficits. These patients need to be evaluated quickly to asure rapid application of treatment. Because of limited resources a costefficient and reliable preclinical evaluation is necessary. The APNOESCREEN I® (AS) is a device to monitor patients in whom a SRBD is suspected. The AS was validated against standardised polysomnography (PSG) in a group of 30 patients suffering from typical SRBDs related symptoms. An apnea index (AI) of 15.8±21.8, a hypopnea index (HI) of 11.8±11.3 and an apnea/hypopnea index (AHI) of 27.6±27.1 in the PSG corresponded to 25.4±25.2 breathing pauses/estimated hour of sleep in the AS. There was a strong correlation between the number of breathing pauses/estimated hour of sleep in the AS and the AI (r=0.96; p≤0,01) as well as the AHI (r=0.97; p≤0.01). The sensitivity to detect a SRBD was 94% for a cut off score of AHI>10 as well as for a cut off score of AHI>20. The specificity was 92% for an AHI>10 and 100% for AHI>20. In conclusion the AS is a good and reliable device for patients with a suspected SRBD.
