Singlecentererfahrung mit der NavX-gestützten linksatrialen Substratmodifikation bei Patienten mit Vorhofflimmern

Single-center experience with NavX-assisted left atrial substrate modification in patients with atrial fibrillation

Zusammenfassung

Seit der Einführung von interventionellen Verfahren zur Behandlung des Vorhofflimmerns bedeuten diese für immer mehr Patienten eine echte Therapiealternative. Um die Effektivität der Eingriffe zu steigern, werden sowohl die Kathetertechnologie als auch diverse Mappingverfahren kontinuierlich weiterentwickelt. Dreidimensionale Navigationssysteme, wie das NavX™ (St. Jude Medical, MN, USA) werden zur Verbesserung der Orientierung im Vorhof sowie zur Präzisierung der Läsionen bei gleichzeitiger Reduktion der Strahlungszeit eingesetzt. In einer neuen Version des Systems kann ein vorher angefertigtes CT-/MRT-Bild des linken Vorhofes integriert werden. Die Navigation und die Ablation erfolgt dann mithilfe dieses originalgetreuen Bildes. 100 Patienten wurden ohne CT-Integration behandelt, bei weiteren 20 konsekutiven Patienten wurde das NavX™-System mit CT-Fusion eingesetzt. Lediglich die Prozedurdauer war in dieser kleineren Gruppe geringfügig erhöht (147±34 min vs. 167±39 min, p=0,016), was am ehesten auf die Lernkurve zurückzuführen ist. Bei allen anderen untersuchten Parametern (Anzahl Energieabgaben, Gesamtenergie, Durchleuchtungszeit) sind keine Unterschiede zwischen den Behandlungsgruppen festgestellt worden. Nach einer mittleren Beobachtungszeit von 16,5 Monaten waren 60% aller behandelten Patienten (68% von den Patienten mit paroxysmalem Vorhofflimmern und 50% der Patienten mit permanentem Vorhofflimmern) im stabilen Sinusrhythmus.

Abstract

Since the introduction of interventional treatment for atrial fibrillation, this treatment has become more and more considerable for a significant patient group. In order to increase the efficacy of the ablation procedure, 3D navigation systems such as the NavX™ (St. Jude Medical, MN, USA) system are widely used. With the use of a nonfluoroscopic mapping system, navigation in the left atrium, lesion establishment can be advanced and fluoroscopy time can be significantly reduced. In an updated version of the system, integration of a previously recorded CT/MRT-scan of the left atrium was implemented. Consequently, the navigation and the ablation procedure can be performed using a 1 : 1 copy of the left atrium. The ablation procedure was performed in 100 patients without the CT-fusion feature, in 20 consecutive patients the NavX™-fusion were used. Only the procedure duration slightly increased within this small group (147±34 min vs 167±39 min, p=0.016) which can be put down presumably to the learning curve. All other investigated parameters (number of radiofrequency applications, delivered energy, x-ray duration) did not show a significant difference among the examined groups. After the mean follow-up of 16.5 months 60% of the patients (68% success in patients with paroxysmal atrial fibrillation and 50% success in patients with permanent atrial fibrillation) were in stable sinus rhythm.
