Summary

The mortality rate of acute renal failure (ARF) after cardiac surgery is still high. The incidence and prevalence of ARF rises due to older patients and the therefore related comorbidities. In this comparative study of two patient collectives, we were able to show that beginning renal replacement therapy earlier significantly reduces the mortality of postoperative patients with ARF. The incidence of complications related to ARF – sepsis, reintubation, intensive care stay, duration of ARF – were also significantly lowered by starting renal replacement therapy earlier.

Zusammenfassung

Das postoperative akute Nierenversagen (ARF) in der Herzchirurgie hat nach wie vor eine hohe Mortalität. Durch die Zunahme des Patientenalters und der damit assoziierten Komorbiditäten tritt das ARF mit konstanter oder sogar steigender Prävalenz und Inzidenz auf. In der vorliegenden Untersuchung konnte im Vergleich zweier Patientenkollektive gezeigt werden, dass der frühzeitige Einsatz einer Nierenersatztherapie die Mortalität der Patienten senkt. Gleichzeitig konnte die Rate an mit dem ARF verbundenen Komplikationen – Sepsis, Reintubation, Intensivaufenthaltsdauer, Dauer des ARF – durch die frühzeitige Nierenersatztherapie signifikant gesenkt werden.
