Zusammenfassung
Eine erhebliche Anzahl schizophren Erkrankender ist bereits vor Krankheitsbeginn in ihrer Persönlichkeitsstruktur und ihrem Verhalten psychisch auffällig, sowohl in introversiv-schizoider Weise wie auch in einer eher expansiv-unstrukturierten Weise. Ein erheblicher Anteil später straffälliger Kranker entstammt zudem sozial sehr randständigen Verhältnissen. Es überlagert sich somit eine früh manifeste Intentionalitätstörung mit einem Mangel an positiven Sozialerfahrungen. Das klinische Bild des Jugendlichen kann aber zunächst als markante „Störung des Sozialverhaltens“ imponieren. Schon 1920 hat die schweizerische Psychiaterin Gertrud Rinderknecht Jugendliche beschrieben, bei denen in besonderer Weise Straffälligkeit mit der Symptomatik einer psychotischen Erkrankung verschränkt ist; sie hat das „kriminelles Heboid“ genannt. Sie plädierte dafür, diese Personen als Kranke anzusehen, auch wenn die Delinquenz nicht Ausdruck markanter schizophrener Akutsymptomatik war, sondern Ausdruck eines basalen Strukturmangels. Sie steht damit im Einklang mit der weiteren Forschung zu schizophrenen Prodromalstadien. In der Versorgungsrealität für psychisch gestörte Rechtsbrecher ist dieses Wissen, dass Störungen des Sozialverhaltens eine psychotische Basis haben können, aber nur schwach verankert. Immer wieder gibt es die Neigung, die psychotischen Zustände als drogeninduzierte Krisen bei dissozialer und emotional-instabiler Persönlichkeitsstörung zu bagatellisieren und die Patienten erziehen zu wollen, statt sie adäquat antipsychotisch zu behandeln.
Abstract
A considerable proportion of people with schizophrenia already become mentally conspicuous in their personality structure and behavior even before onset of the disease, in an introversive schizoid manner as well as a more expansive unstructured manner. A considerable proportion of sufferers who later become delinquent emanate from very borderline social circumstances. There is therefore an overlap of early manifestation of an intentionality disorder with a lack of positive social experiences. The clinical picture of adolescents can initially present as a marked “disorder of social behavior”. As early as 1920 the Swiss psychiatrist Gertrud Rinderknecht described adolescents in whom delinquency is intertwined in a special manner with the symptoms of a psychotic disease, which she named the “heboid criminals”. She advocated that these persons should be considered as being ill even if the delinquency was not the expression of an acute marked schizophrenic syndrome but expression of a lack of basal structure. This is therefore in conformity with further research on prodromal stages of schizophrenia. In the treatment reality of mentally disturbed offenders, the knowledge that disorders of social behavior can have a psychotic basis, is only weakly anchored. The tendency to trivialize psychotic conditions as drug-induced crises in dissocial and emotionally unstable personalities and wanting to train patients instead of giving them adequate antipsychotic treatment continually reoccurs.
