Résumé

Les fistules anales en fer à cheval sont rares mais représentent en règle une situation difficile sur le plan thérapeutique. Elles sont associées à une morbidité certaine et à un risque significatif d’incontinence postopératoire. Le 1er temps opératoire consiste à drainer le trajet transversal et les temps suivants s’adressent au trajet fistuleux principal. Les techniques d’épargne sont envisageables mais n’ont pas fait l’objet d’une évaluation spécifique dans ce contexte précis.

Abstract

Horseshoe extensions in anal fistulas are uncommon but represent a challenging condition for their management. They are associated with substantial morbidity and carry a significant risk of anal incontinence. First operative time needs to address the drainage of the horseshoe extension and following operations are for main fistula track. Sphincter sparing techniques are conceivable but their efficacy has not been specifically assessed in this situation.
