Un accident ischémique protubérantiel révélé par une paupière douloureuse

A pontine stroke revealed by a painful eyelid

Résumé

Introduction

La douleur oculaire représente une manifestation rare des accidents vasculaires cérébraux (AVC).

Observation

Nous rapportons le cas d’un patient âgé de 65 ans qui a présenté un accident ischémique latéroprotubérantiel postérieur gauche (exploré par imagerie encéphalique et électrophysiologie), dont la manifestation initiale était une douleur de la partie postérieure de la paupière inférieure gauche, complétée par des troubles sensitifs controlatéraux épargnant la face.

Conclusion

Il s’agissait d’une forme peu fréquente de syndrome alterne du tronc cérébral.

Introduction

Ocular pain is an uncommon sign of a stroke.

Case report

We report the case of a 65-year-old patient who presented with an ischemic left dorsolateral pontine stroke (studied with brain imaging and electrophysiological tests), whose symptoms were pain in the posterior lower left eyelid followed by contralateral hemi-paresthesias which did not affect the face.

Conclusion

This case illustrates that eyelid pain may be an initial symptom of vertebrobasilar stroke.
