Key Points

The conversion of Roux-en-Y gastric bypass to SADI-S/duodenal switch has a high rate of complications.

Most complications are leakage or stricture of the gastric tube at the gastro-gastrostomy site due to ischemia after crossing the staple lines.

We propose to stage the procedure, restoring gastric anatomy and performing duodenoileostomy in the first step; in the second step, sleeve gastrectomy is delayed by at least 6 months to allow new vascularization over the suture lines.
