Résumé
Les enfants de mères obèses de faible niveau socioéconomique ont un risque important de développer une surcharge pondérale. Certains comportements éducatifs sont susceptibles de majorer ce risque. Le but de ce travail est d’étudier l’influence d’un programme d’éducation thérapeutique chez des femmes obèses sur leur attitude éducative. Quarante-six femmes obèses ou en surpoids, mères d’au moins un enfant de moins de 12 ans, participant à un cycle d’éducation thérapeutique en centre social entre octobre 2011 et février 2013, ont été incluses. De plus, 5 femmes ont fait l’objet d’un entretien semi-directif visant à évaluer leurs pratiques et attitudes. A l’issue du cycle, les mamans obligeaient moins souvent leur enfant à finir leur assiette (p=0,046) et leur demandaient plus souvent s’ils avaient faim avant de leur donner un goûter (p=0,015). Les familles jouaient plus souvent aux jeux de société ensemble (p=0.049). Les entretiens confirmaient une réelle modification des comportements éducatifs sur le plan alimentaire et de l’activité physique. Cette étude suggère que dans les milieux défavorisés, la prise en charge précoce de femmes obèses, mères de famille, favorise le changement de comportement au niveau familial, et pourrait être un levier dans la prévention de l’obésité de l’enfant.
Abstract
Obese mothers of low socioeconomic status have children with a significantly higher risk of developing obesity. Certain up-bringing practices seem to increase this risk. The purpose of this research is to investigate the influence of patient education in obese women on their up-bringing behavior. Forty-six overweight or obese women, mothers of at least one child under 12, taking part in a social center patient education program betweenOctober 2011 and February 2013 were included. Asemi-structured interviewwas conductedwith 5women to assess their practices and behaviors. At the end of the patient education program, the mothers forced their children less often to finish their plates and asked them more often if they were hungry before giving them a snack. Families often played board games. The interviews confirmed a substantial change in the up-bringing practices concerning attitude towards food and exercising. This study suggests that in social-economically disadvantaged areas, early management of obese mothers promotes behavioral change at the household level, and could be a lever in the prevention of childhood obesity.
