Résumé

La dilatation œsophagienne est un problème classique après implantation cœlioscopique d’un anneau modulable gastrique pour obésité. À la différence de la dilatation de poche gastrique ou de l’érosion, il ne s’agit d’une complication de la technique que dans les rares cas sévères nécessitant l’ablation de l’anneau, les autres cas pouvant être gérés par la désinsufflation sous contrôle radiologique. Nous proposons une classification des cas de dilatation en quatre stades et un traitement adapté à chacun, le recours ultime étant la conversion en by-pass gastrique sous cœlioscopie.

Abstract

Oesophageal dilatation is a problem that must be taken into account when following up laparoscopic gastric banding. We suggest distinguishing this issue from pouch dilatation and gastric erosion and considering it uniquely a complication of band removal in severe cases. Most cases can be handled by deflating the band under radiological monitoring. We describe four stages of dilatation and the relevant treatment for each, the ultimate alternative being laparoscopic gastrojejunal by-pass.
