Traumatische Ruptur der Sehne des M. latissimus dorsi bei einem Kampfsportler

Traumatic rupture of the latissimus dorsi tendon in a martial artist

Zusammenfassung

Bei einem 24-jährigen Kampfsportler zeigte sich eine traumatische Ruptur der Sehne des M. latissimus dorsi. Klinisch war die Innenrotation gegen Widerstand bei 90° abduziertem Arm abgeschwächt und ein Pseudotumor in der Axilla palpabel. Magnetresonanztomographisch zeigte sich eine 5 cm retrahierte Sehne des M. latissimus dorsi. Die Exposition und Mobilisation des Sehnenstumpfes erfolgte über einen axillären Zugang und die Refixation über einen subpektoralen Zugang. Die Sehne wurde mittels Fadenankern am Humerus refixiert. Postoperativ erfolgte eine kurzfristige Immobilisation und anschließend schrittweiser Freigabe der Beweglichkeit.

Abstract

A 24-year-old martial artist presented with a traumatic rupture of the latissimus dorsi muscle tendon. During physical examination “high internal rotation” and retroversion of the arm was weakened and a pseudotumor in the axilla was palpable. Magnetic resonance imaging showed a 5 cm retracted tendon stump of the latissimus dorsi muscle. During surgical refixation the tendon stump was exposed und mobilized via an axillary approach and the refixation was performed via a subpectoral approach. The tendon was refixed to the humerus using suture anchors. The postoperative protocol included short-term immobilization with restriction of combined abduction and external rotation and then gradual increase of mobility.
