Zusammenfassung
Die Alkaptonurie (Ochronose) ist eine autosomal-rezessiv vererbbare Störung des Tyrosinstoffwechsels. Mangelnder Abbau der Aminosäure Tyrosin verursacht eine Anreicherung der Homogentisinsäure. Die Oxidation der Homogentisinsäure führt ab der 4. Dekade zu Ablagerungen des schwarzen ochronotischem Pigments im hyalinen Gelenkknorpel und dem gefäßarmen Bindegewebe und verursacht progrediente degenerative Gelenkveränderungen. Der Fallbericht beschreibt einen 47-jährigen männlichen Patient, der trotz hochdosierter Schmerzmedikation und intensiver konservativer Therapie über Schmerzen des linken Schultergelenks klagte. Nach einer initialen Arthroskopie mit Nachweis einer fortgeschrittenen Arthrose und grauschwarzen Knorpel- und Synovialgewebe wurde eine anatomische Schulterprothese implantiert. Die Diagnose Alkaptonurie wurde anhand des Laborbefunds mit 1,7 g/l Homogentisinsäure im Urin gestellt. Postoperativ konnten ein gutes Bewegungsausmaß sowie eine Schmerzlinderung erzielt werden. Die Röntgenkontrolle zeigte einen regelrechten Sitz des Implantats. Der Fallbericht beschreibt das seltene Krankheitsbild Alkaptonurie mit einer Erstmanifestation am Schultergelenk. Die ausgeprägte Symptomatik konnte durch die Implantation der Schulterprothese adäquat behandelt werden.
Abstract
Alcaptonuria (ochronosis) is an autosomal recessive genetic disorder of tyrosine metabolism. Impaired degradation of the amino acid tyrosine causes accumulation of homogentisic acid. From the 4th decade onwards oxidation of homogentisic acid leads to deposition of ochronotic pigment in hyaline articular cartilage and poorly vascularized connective tissue. The result is progressive generalized joint degeneration. A 47-year-old man reported intensive pain in the left shoulder despite high doses of analgesics and conservative therapy. Physical examination revealed restricted mobility at all levels due to pain. Initial shoulder arthroscopy showed advanced degeneration of the humerus and glenoid, with grey–black pigmented cartilage. Laboratory findings of 1.7 g/l urinary homogentisic acid confirmed the diagnosis of alcaptonuria. Because of the advanced degeneration of the left shoulder, total shoulder arthroplasty was carried out. Improved range of motion and pain relief could be achieved postoperatively. X‑rays showed proper positioning of the implant. This case report describes the rare disease of alcaptonuria with affection of the shoulder. The pronounced symptoms were treated successfully by anatomic shoulder replacement.
