Zusammenfassung
Das distale Radioulnargelenk (DRUG) ist elementar für die Gebrauchsfähigkeit der oberen Extremität. Eine Instabilität im DRUG kann sich fatal auswirken. Zahlreiche klinische Testverfahren zur DRUG-Instabilität wurden beschrieben. Es besteht allerdings noch keine Standardisierung der Evaluation einer Instabilität. Die Anwendung und Interpretation der einzelnen Testverfahren ist anwenderspezifisch und die korrekte Diagnosestellung erfordert eine große Erfahrung. Von größerer Bedeutung sind der dorsopalmare Stresstest, das Ulnar-fovea-Zeichen und der Presstest, da diese gegen den Goldstandard der Handgelenkarthroskopie evaluiert wurden und von adäquater Güte sind.
Abstract
The distal radioulnar joint (DRUJ) is crucial regarding the functionality of the upper extremity. DRUJ lesions can significantly limit functionality. Numerous clinical tests have been described to diagnose DRUJ instability. However, no consensus has been reached concerning standardization of the DRUJ instability evaluation. The application and interpretation of these tests strongly depend on the individual examiner and his/her interpretation and experience. The dorsopalmar stress test, the ulna fovea sign and the press test were compared to wrist arthroscopy as the gold standard.
