Zusammenfassung
Die synoviale Chondromatose ist gekennzeichnet durch eine Metaplasie der Synovialmembran, deren Ursache unklar ist. Die Folge des proliferativen Geschehens sind multiple freie kartilaginäre Körper, die intra- und extraartikulär gefunden werden. Es wird der Fall eines 14-jährigen Athleten beschrieben, der wegen einer einseitigen sekundären Chondromatose des Schultergelenkes behandelt wurde. Über eine Arthroskopie, kombiniert mit einem sparsamen anterioren Zugang im Bereich des Sulcus bicipitalis ließen sich sämtliche 14 freie Gelenkkörper entfernen. Auf die Bedeutung der Differenzierung zwischen primärer und sekundärer Chrondromatose bezüglich Therapie und Prognose wird näher eingegangen.
Abstract
Synovial chondromatosis is characterized by a metaplastic process of unknown origin. Proliferation of cartilaginous bodies can lead to multiple intra-articular and extra-articular fragments which are most commonly found in the knee and hip joints. A rare case of a 14-year-old male athlete with monoarticular secondary chondromatosis of the shoulder is presented. Arthroscopy combined with minimally invasive anterior access was performed for complete extraction of all 14 arthroliths. The importance of histologic differentiation between primary and secondary chondromatosis is discussed with respect to therapy and prognosis.
