Posttraumatische Bewegungseinschränkungen des Ellenbogengelenkes

Abstract

The incidence and pathogenesis of elbow stiffness is still unknown. We can distinguish between intrinsic, extrinsic and combined reasons for limited motion of the elbow. Conservative therapy is usually the first choice in the case of post-traumatic elbow stiffness. If this is not successful, operative treatment is necessary. Patient compliance as well as the operation technique are deciding factors in prognosis. The arthrolysis can be performed using arthroscopy, the open technique or distraction arthroplasty. The type of operative treatment and approach are determined by the pathomorphological factors. Prophylaxis for heterotopic ossification is very important.

Zusammenfassung

Die Inzidenz und Pathogenese der Ellenbogengelenksteife ist weitgehend unbekannt. Es werden intrinsische, extrinsische und kombinierte Ursachen der Bewegungseinschränkung unterschieden. Die konservative Therapie ist bei den posttraumatischen Bewegungseinschränkungen am Ellenbogen zunächst die Behandlung der ersten Wahl. Kann hierdurch kein Erfolg erreicht werden, ist die operative Arthrolyse indiziert. Neben der Operationstechnik ist die Patientencompliance von entscheidendem prognostischen Wert. Die Arthrolyse kann in arthroskopischer oder in offener Technik und als Distraktionsarthrolyse erfolgen. Die Art der operativen Behandlung und die Zugangswahl werden durch das pathomorphologische Substrat bestimmt. Eine Prophylaxe heterotoper Ossifikationen ist wichtig.
