Shoulder resurfacing in patients with avascular necrosis and posttraumatic humeral head necrosis

Abstract

Introduction

Shoulder surface replacement arthroplasty has been established for several decades as a mean to restore comfort and function of the shoulder for many afflictions that derange the normal anatomy. Surface replacement may offer some advantages over the stemmed prostheses.

Purpose

The purpose of the study was to evaluate the clinical and radiological results of Copeland cementless surface replacement arthroplasty (CSRA) applied in patients with bony defects of the humeral head.

Patients and methods

The study was conducted on 18 patients (2 patients with chronic posterior dislocation, 8 patients with AVN, 8 patients with posttraumatic head necrosis) with bony head defects that were operated on between 1999 and 2006. The patients were followed up clinically and radiologically for a mean of 24.8 months (range, 10–78 months). There were 8 female and 10 male shoulders. The mean age was 55.3 years (range, 27–71). The clinical assessment was performed with the Constant score.

Results

The Constant score significantly improved from a mean of 14.83 points preoperatively to 64.34 points postoperatively. The average pain score increased from 0.3 points to 9.8 points. The average ROM score increased from 8.21 points to 23.21 points. The humeral offset increased from 22.3 mm to 28.8 mm.

Conclusion

The shoulder surface replacement arthroplasty shows good mid-term results in patients with bony defects.

Einleitung

Der Oberflächenersatz an der Schulter hat sich in den letzten Jahren zunehmend etabliert und scheint in manchen Situationen vergleichbar oder sogar von Vorteil gegenüber der konventionellen endoprothetischen Versorgung an der Schulter zu haben.

Studienziel

Das Ziel der vorliegenden Untersuchung war die klinische und radiologische Evaluation von zementfreien Oberflächenprothesen (Copeland™) im Bereich der Schulter bei ossären Defektzuständen.

Patienten und Methoden

In die Studie wurden 18 konsekutive Patienten mit Humeruskopfdefektzuständen (8 Patienten mit posttraumatischen Kopfnekrosen, 8 Patienten mit avaskulärer Humeruskopfnekrose, 2 Patienten mit chronischer dorsaler Luxation). Es handelte sich um 8 Frauen und 10 Männer. Das mittlere Alter betrug 55,3 Jahre (27–71 Jahre). Die Patienten wurden prospektiv klinisch und radiologisch untersucht. Der mittlere Nachuntersuchungszeitraum betrug 24,8 Monate (10–78 Monate). Die klinische Dokumentation erfolgt anhand Constant-Scores.

Ergebnisse

Der mittlere Constant-Score verbesserte sich von 14,83 auf 64,34 Punkte. Der mittlere Schmerz-Score verbesserte sich von 0,3 Punkten (0–5 Punkte) präoperativ auf einen Wert von 9,8 Punkten (5–15 Punkte). Das mittlere Bewegungsausmaß verbesserte sich im Constant-Score von 8,21 Punkten (2–10 Punkte) präoperativ auf einen postoperativen Wert von 23,21 Punkten (2–32 Punkte). Das humerale Offset nahm von 22,3 mm präoperativ auf 28,8 mm postoperativ zu.

Fazit

Der Oberflächenersatz an der Schulter bei Patienten mit ossären Defektzuständen im kurzen und mittelfristigen Verlauf erbringt gute und akzeptable klinische Ergebnisse.
