Ergebnisse der Resektionsarthroplastik an den Langfingergrundgelenken mit Interposition der palmaren Platte bei nichtrheumatoider Arthritis

Results of a modified proximally based volar plate resection arthroplasty of the metacarpophalangeal joint in non rheumatoid arthritis

Summary

This retrospective study shows the results of 6 patients, whose metacarpophalangeal joints have been modified by resection arthroplasty with the interposition of the proximally based palmar plate with sparsely partial resection of the metacarpal head without cutting the collateral ligaments. Indications were posttraumatic, idiophatic, and in one case a tumorous arthrosis of the MP joint.

After a medium period of 27 months, the range of motion improved from a median of 40° to a median of 80° without extension deficits – along with a pinch strength of 5.5 kg (opposite side 7.3 kg). All patients were without trouble at a dash level of 8. These results are compared to other published resectional arthroplasty studies with the interposition of autologue tissue, joint transplantation and prothesis implantation with comparable indications.

Zusammenfassung

In einer retrospektiven Studie werden die Ergebnisse von 6 Patienten dargestellt, bei denen eine modifizierte Resektionsarthroplastik der Langfingergrundgelenke mit Interposition der proximal gestielten palmaren Platte bei sparsamer Teilresektion des Mittelhandköpfchens ohne Durchtrennung der Seitenbänder durchgeführt wurde. Indikationen waren posttraumatische, idiopathische oder tumoröse Destruktionen der Langfinger-Grundgelenke.

Nach einem mittleren Nachuntersuchungszeitraum von 27 Monaten verbesserte sich das Bewegungsausmaß von im median 40° auf 80° ohne Streckdefizit bei einer Spitzgriffkraft von 5,5 kg (Gegenseite 7,3 kg) Alle Patienten waren beschwerdefrei bei einem DASH Wert von 8. Diese Ergebnisse werden mit publizierten Studien von Resektionsarthroplastiken mit Interposition autologen Gewebes, Gelenktransplantationen sowie Prothesenimplantationen bei vergleichbaren Indikationen verglichen.
