Objectif
Étudier la faisabilité d’un programme d’éducation thérapeutique (ETP) à la prévention des chutes après un accident vasculaire cérébral en médecine physique et de réadaptation.
Matériels et méthodes
Ce programme est construit selon le guide méthodologique élaboré par le groupe SOFMERETP-AVC. Il a bénéficié d’une autorisation de l’Agence Régionale de santé Languedoc-Roussillon. Il est proposé à toute personne présentant des séquelles d’AVC et à ses aidants selon une approche biopsychosociale. Il comporte des activités individuelles permettant l’acquisition d’apprentissages, et des séances collectives visant les échanges de stratégies d’adaptation. Le patient est évalué ainsi que le processus d’ETP.
Résultats
Cinquante-trois patients ont été inclus du 1er juin 2011 au 30 décembre 2012. Le délai post-AVC était de 2,8 mois (15 jours–11 mois) pour 41 patients ; 12 autres patients avaient une hémiplégie chronique. Tous avaient des antécédents de chutes. En fin de programme, 42 patients (85 %) avaient atteint les objectifs éducatifs fixés par le bilan éducatif. L’évaluation entre quatre et six mois montrait que neuf patients avaient chuté au moins une fois. Un patient a été victime d’une chute traumatique.
Conclusion
Les difficultés d’évaluation d’interventions complexes non pharmacologiques comme l’ETP, a fortiori sur chez des patients en post-AVC présentant des troubles cognitifs, n’ont pas permis d’exploiter l’ensemble des résultats. Des études sont nécessaires sur de plus grandes cohortes pour déterminer l’intérêt et le coût de ce type de programmes en post-AVC.
Abstract
Objective
To identify the feasibility of a therapeutic education program to prevent falls after a stroke during inpatient rehabilitation.
Methods
The present program is built according to the methodological guidelines proposed by the SOFMER. It has been validated by the ARS.
Inclusion criteria
Post-stroke inpatients and their family caregivers. The program includes individual activities for learning tasks and group activities for the exchange of coping strategies. Patients, as well as the learning process, are then assessed.
Results
Fifty-three inpatients admitted in our rehabilitation hospital were included in the program during the period between 1st June 2011 and 30th December 2012. Last stroke occurrence ranged from 14 days to 12 months (mean, 2.8 months) for 41 patients. Twelve patients had a chronic hemiplegia. All had histories of falling. At the end of the program, 42 patients (85%) acquired theoretical and practical skills determined by the educational diagnosis. Nine patients reported falls between 4 and 6 months after the end of the program. A traumatic injury after fall was described in one patient.
Conclusion
Evaluations of multifactorial interventions likes TPE, with cognitive impaired stroke survivors, were particularly difficult to perform. TPE justifies larger programs to further assess efficacy and cost effectiveness.
