Résumé
L’augmentation de l’espérance de vie relative à l’amélioration de la qualité des soins a permis aux enfants atteints de paralysie cérébrale de devenir adultes malgré la lourdeur de leur pathologie. La mise en place d’une transition enfant-adulte et d’un suivi adulte s’est révélée nécessaire. L’adolescence est une période de transition vers la vie adulte qui comporte des risques spécifiques et qui vient marquer la pérennité du handicap. Une préparation à la transition vers une prise en charge de l’adulte doit s’effectuer précocement. La mise en place, auprès du jeune et de sa famille, d’une information adaptée est indispensable. Une consultation de transition et une bonne qualité de transmission de l’information permettent une meilleure adaptation au suivi en milieu adulte. Le suivi adulte est nécessaire en MPR pour coordonner le suivi médical et rééducatif idéalement sur une consultation spécifique de la pathologie. Chez l’adulte, nous aborderons les problématiques spécifiques de l’installation assise, de la scoliose neurologique, de la chirurgie multiétagée, de la prise en charge de la dystonie axiale et de la myélopathie cervicale, des complications dégénératives articulaires précoces et de la possibilité des chirurgies prothétiques aux membres inférieurs.
Abstract
The increase in life expectancy due to improvements in the quality of care has meant that children with cerebral palsy reach adulthood despite the burden of their illness. The implementation of a child to adult transition and adult monitoring has proven necessary. Adolescence is a transitional period towards adult life that has specific risks associated with it, and which marks the progression of the condition. Preparing for the transition towards adult care must take place as early as possible. Providing the child and their family with all the relevant information is essential. A transition meeting and good quality information transfer ensures that appropriate adjustments can be made to monitoring in an adult environment. Adult level monitoring is required in PM & R to coordinate medical and rehabilitation follow-ups, ideally in an illness specific consultation appointment. For adult patients, we will discuss the specific problems of the seating position, the neurological effects of scoliosis, multi-stage surgical procedures, the treatment of axial dystonia and cervical myleopathy, early degenerative complications to the joints and the possibility of prosthetics to the lower limbs.
