Résumé
Le syndrome dysexécutif est observé dans de nombreuses pathologies, il est l’une des séquelles les plus fréquentes du traumatisme crânien sévère. Il est très invalidant et incomplètement évalué par les tests neuropsychologiques classiques « papier crayon », de par leurs conditions de passation très structurées. Des évaluations dites « écologiques » ont été développées et nous en présentons certaines, en général réalisées par des ergothérapeutes. Il s’agit d’évaluations effectuées en situations proches de la vie quotidienne ou même d’évaluations réalisées au domicile du patient. Nous avons développé un test d’évaluation de l’exécution d’une tâche de cuisine en ergothérapie, dans laquelle le patient doit préparer un gâteau au chocolat et une omelette. Nous avons décrit une classification des erreurs en deux niveaux, l’un purement descriptif, l’autre « neuropsychologique », prenant en compte les conditions dans lesquelles les erreurs ont été commises. Dans notre étude, nous avons pu montrer que les patients effectuaient significativement plus d’erreurs en exécution réelle de la tâche que lors de sa simple planification écrite. Les erreurs survenaient le plus souvent en raison d’une mauvaise interaction du sujet avec son environnement (absence de prise en compte du contexte, adhérence à l’environnement). Cette épreuve est très sensible à un syndrome dysexécutif et la mise en situation réelle, dans des conditions de passation très précises, permet d’objectiver les difficultés des patients n’apparaissant pas dans les tests classiques. D’autres outils se développent et sont à utiliser dans le milieu ordinaire de vie du patient, orientant l’évaluation sur l’activité et la participation de l’individu.
Abstract
Dysexecutive syndrome occurs in various pathologies; it is a frequent sequela of severe traumatic brain injury. It causes severe disabilities and it is incompletely assessed by the classical “paper and pencil” neuropsychological tests, probably because of the highly structured conditions in which they are conducted. Therefore, “ecological” assessments are being developed. Ideally, such assessments should use situations that closely resemble real life situations, or even assess the patient in his/her own home. We have developed an ecological assessment performed in the therapeutic kitchen of the occupational therapy department. Subjects are required to actually perform a cooking task: they have to make a chocolate cake and an omelette. We described a classification of errors at two levels, the purely descriptive and the “neuropsychological”, taking into account the mechanisms leading to the errors’ occurrence. In our study, we demonstrated that patients made significantly more errors in actual task performance, than in the written planning of the task (script generation). Errors occurred mostly because of a pathological interaction by patients with their environment (lack of context analysis, environmental adherence). This task is very sensitive to dysexecutive syndrome, and the use of a real life situation enables us to measure difficulties that do not appear in the usual neuropsychological tests. Other tools have being developed for use in the patient’s everyday life, with the purpose of guiding assessment of the individual’s activity and participation.
