Résumé

Les lésions radio-ulnaires distales traumatiques sont fréquentes. Elles sont secondaires à une fracture de l’épiphyse distale du radius et/ou de l’ulna. Le diagnostic dans ce cas pose peu de problème si les radiographies sont correctement analysées. Le traitement est chirurgical en cas de déplacement. Il consiste à réduire les fractures et à mettre en place une synthèse par broches ou par plaque. Cependant, une réduction imparfaite ou un déplacement secondaire peut entraîner un cal vicieux responsable d’une limitation de la pronosupination, de douleurs et d’une arthrose secondaire. Dans ce cas, un traitement chirurgical est le plus souvent nécessaire. Ce traitement peut être conservateur par restauration de l’anatomie initiale ou un traitement palliatif afin de supprimer les contraintes anormales.

Abstract

Distal radio-ulnar joint injuries are common. Most of the time, these injuries are secondary to distal radius or ulna fractures. Diagnosis is generally made with standard X-rays. Surgery is required in cases of distal radioulnar joint or displaced fractures. In such cases, fracture reduction and osteosynthesis, with pins or plate, are performed. However, malunion may occur in cases of imperfect reduction or postoperative displacement, leading to pain, limited pronosupination and distal radio-ulnar joint osteoarthritis. In such cases, conservative or palliative surgical treatment is often necessary.
