Zusammenfassung
Technische Entwicklungen im Bereich der bildgebenden Verfahren der Dermatologie ermöglichen heutzutage eine hochauflösende Darstellung von Hautveränderungen, z. T. auf zellulärer Ebene. Hervorzuheben ist dabei eine sofortige, schmerzfreie In-vivo-Befunderhebung. Hierdurch sind auch serielle Aufnahmen zur Beurteilung eines Therapieansprechens (z. B. Therapieverlauf einer photodynamischen Therapie bei aktinischen Keratosen) im Sinne eines „imaging-guided monitoring“ möglich. Durch fortschreitende Transformation im Gesundheitswesen, verbunden mit Zunahme der Digitalisierung, Steigerung der Netzwerkgeschwindigkeiten und Kapazität der Datenspeichersysteme sowie Entwicklung neuer Algorithmen, werden der Integration der künstlichen Intelligenz (KI) in die tägliche Praxis zahlreiche neue Möglichkeiten eröffnet. Dabei soll die „digitale Biopsie“ in vivo nicht als Alternative oder Konkurrenz zur konventionellen histopathologischen Aufarbeitung verstanden werden, sondern vielmehr eine Vorauswahl von auffälligen Hautveränderungen treffen. Dadurch können die diagnostische Genauigkeit von Hauttumoren gesteigert werden und unnötige Exzisionen vermieden werden, ohne dabei behandlungsbedürftige Hautveränderungen zu übersehen.
Abstract
Technical innovations in imaging techniques in dermatology nowadays enable an immediate, pain-free in vivo assessment of skin pathologies at high resolution, in some case at the cellular level. This also enables serial recordings to assess the response to treatment (e.g., during the course of photodynamic therapy for actinic keratoses) in the sense of “imaging-guided monitoring.” The ongoing transformations in healthcare combined with the increase in digitization, increases in network speeds and the capacities of data storage systems, and the development of new algorithms are opening numerous new possibilities for integrating artificial intelligence into everyday practice. Therefore, in vivo “digital biopsy” should not be understood as an alternative or competition to conventional histopathological workup, but rather as a preselection of conspicuous skin changes. This can increase the diagnostic accuracy of skin tumors and avoid unnecessary excisions without overlooking skin changes that require treatment.
