Zusammenfassung
Die Weltgesundheitsorganisation (WHO) definiert Anämie als Verminderung des Hämoglobins, bei Frauen <12 g/dl und bei Männern <13 g/dl. Das Wissen über die Pathophysiologie der Erythropoese mit molekulargenetischen Veränderungen, der zentralen Rolle von Hepcidin im Eisenstoffwechsel und der Bedeutung des „zellulären O2-Sensors“ nimmt stetig zu. Dies ermöglicht eine rationale Differenzialdiagnostik, die Voraussetzung für eine Behandlung sein sollte. Die Basisdiagnostik besteht aus einer sorgfältigen Anamnese, einem großen Blutbild, C‑reaktivem Protein (CRP), Ferritin und löslichen Transferrinrezeptoren zur Suche nach der Anämieursache. Zur Therapie stehen derzeit neben der Behandlung der Grunderkrankung nur Vitamine, orale und intravenöse Eisenpräparate sowie erythropoesestimulierende Agenzien (ESA) zur Verfügung. Eine frühzeitige Therapie kann Erythrozytentransfusionen als Ultima Ratio vermeiden, und in naher Zukunft werden noch weitere zielgerichtete Medikamente erwartet.
Abstract
Anemia is defined by the World Health Organization (WHO) as a decrease in hemoglobin to <12 g/dl in women and <13 g/dl in men. Knowledge of the pathophysiology of erythropoiesis in the presence of molecular genetic changes, the central role of hepcidin in iron metabolism, and the relevance of the “intracellular O2 sensor” is continuously increasing. This enables a rational differential diagnosis, which is a prerequisite for treatment. Basic diagnostics comprise careful medical history taking, complete blood count, C‑reactive protein (CRP), ferritin, and soluble transferrin receptor to identify the cause of anemia. In addition to treatment of the underlying disease, vitamins, oral and intravenous iron, and erythropoiesis-stimulating agents (ESA) are the only therapeutic options. Early treatment can avoid the necessity of a transfusion of packed red blood cells, which should be a last resort. New targeted therapies are expected in the near future.
