Zusammenfassung
Klonale Hämatopoese ist definiert als Auftreten von Mutationen in Leukozyten des peripheren Bluts bei nicht hämatologisch erkrankten Individuen. Diese Sequenzveränderungen betreffen oft Gene, die wichtige Treibergene, z. B. DNMT3A und TET2, für hämatologische Neoplasien (hN) wie myelodysplastische Syndrome (MDS), myeloproliferative Neoplasien (MPN) und akute myeloische Leukämien (AML) darstellen. In diesem Zusammenhang wurde der Begriff der CHIP – klonale Hämatopoese unbestimmten Potenzials – geprägt. Der Nachweis von CHIP-Mutationen ist mit einem erhöhten Risiko für kardiovaskuläre Erkrankungen und hN assoziiert. Der Begriff CCUS – klonale Zytopenie unklarer Signifikanz – bezeichnet Situationen, in denen bei unklaren Zytopenien ohne signifikante Dysplasien dennoch klonale Mutationen nachweisbar sind. So stellen CCUS Vorstufen für MDS dar. Die Evolution von CHIP bis zu MDS oder sekundärer AML ist geprägt von Zunahme der Klongröße, Akquisition weiterer Mutationen und Einfluss wichtiger Kofaktoren wie der Knochenmarksnische, Immunsurveillance, Inflammation u. a. Das Risiko des Übergangs einer Zytopenie in eine hämatologische Systemerkrankung hängt ab von der Art und Anzahl der Mutationen. Der Nachweis von CHIP ist prädiktiv für das Entstehen von MDS und AML, z. T. Jahre vor Ausbruch der Erkrankungen, und assoziiert mit dem Risiko für und der Prognose von therapieassoziierten myeloischen Neoplasien. Auch ist CHIP relevant für aplastische Anämien, chronisch lymphatische Leukämien (CLL), maligne Lymphome und solide Tumoren. Bezogen auf die Gesamtpopulation werden die meisten Personen mit Nachweis einer CHIP zwar niemals eine hämatologische Systemerkrankung entwickeln, weisen aber dennoch eine erhöhte Gesamtmortalität auf.
Abstract
Clonal hematopoiesis is defined as the occurrence of mutations in peripheral blood leukocytes in individuals without hematologic disease. These sequence changes often affect important driver genes, e.g., DNMT3A and TET2, of hematologic neoplasms such as myelodysplastic syndrome (MDS), myeloproliferative neoplasms (MPN), and acute myeloid leukemia (AML). The term CHIP—clonal hematopoiesis of indeterminate potential—has become established in this context. The demonstration of CHIP mutations is associated with an increased risk of cardiovascular disease and hematologic neoplasms. The term CCUS—clonal cytopenia of undetermined significance—describes situations of unclear cytopenia without significant dysplasia in the presence of clonal mutations. CCUS represent precursor stages of MDS. The evolution of CHIP to MDS or secondary AML is characterized by an increase in the clonal population, acquisition of further mutations, and the influence of important cofactors such as the bone marrow niche, immune surveillance, inflammation, etc. The risk of progression from cytopenia to systemic hematologic disease depends on the type and number of mutations. Evidence of CHIP is predictive for development of MDS and AML, sometimes years before disease onset, and is associated with the risk for and prognosis of treatment-associated malignant neoplasms. CHIP is also relevant for aplastic anemia, chronic lymphatic leukemias, malignant lymphomas, and solid tumors. In terms of the overall population, most individuals with evidence of CHIP will never develop a systemic hematologic disease; they do, however, have increased overall mortality.
