Zusammenfassung
Myelodysplastische Syndrome (MDS) sind maligne Stammzellerkrankungen, die meist im höheren Lebensalter anlässlich der Abklärung einer Zytopenie diagnostiziert werden und sehr unterschiedliche Krankheitsverläufe haben können. Die Diagnostik erfolgt mittels Zytologie, Zytogenetik und Histologie unter Berücksichtigung der Ausprägung der Dysplasien, des Blastenanteils in Blut und Knochenmark und zytogenetischer Befunde nach den Vorschlägen der WHO-Klassifikation. Die Abschätzung der Prognose erfolgt mithilfe der revidierten Version des internationalen Prognosescores (IPSS-R), der neben Blastenanteil und zytogenetischen Befunden auch Blutzellwerte berücksichtigt und 5 Risikogruppen definiert. Niedrigrisikopatienten werden mit dem Ziel der Lebensqualitätsverbesserung mit Transfusionen, ggf. Eisenchelation, ggf. Erythropoetin, ggf. Lenalidomid oder in klinischen Studien behandelt. Hochrisikopatienten im Alter < 65–70 Jahre und in gutem Allgemeinzustand sollte eine allogene Stammzelltransplantation mit kurativer Intention angeboten werden. Hochrisikopatienten, auch im hohen Alter, können mit 5-Azacytidin behandelt werden.
