Das Ovarialkarzinom stellt sich trotz verbesserter Diagnostik und Therapie nach wie vor als ein Problemkarzinom dar. Häufig werden Frühsymptome nicht erkannt, sodass die Diagnose erst in fortgeschrittenen Stadien erfolgt. Ziel der operativen Therapie ist neben dem Erreichen der Tumorfreiheit das exakte Tumorstaging. Es gibt bis heute keine apparative diagnostische Maßnahme, die das operative Staging ersetzen könnte. Insbesondere die Beurteilung des Lymphknotenbefalls ist nur über die Operation möglich. Ein besonderes Vorgehen erfordert die Behandlung des Ovarialkarzinoms bei Erhaltung der Fertilität bei jungen Frauen. Bis auf wenige Ausnahmen erhalten alle Patientinnen eine postoperative Chemotherapie.
