Résumé
La manométrie anorectale (MAR) est un outil d’évaluation largement utilisée dans les troubles anorectaux (TAR) fonctionnels (dyschésie, incontinence fécale) et neurologiques. Nombre de paramètres (pressions de repos, perception du besoin, synchronisme abdominopérinéal) obtenus lors de cet examen sont validés en termes de reproductibilité et de valeur normative au sein de chaque laboratoire, sans qu’aucune valeur ne soit cependant universellement acceptée. Il en est de même pour le réflexe rectoanal inhibiteur (RRAI), élément essentiel de cette exploration manométrique anorectale, puisque permettant de vérifier l’intégrité de l’innervation intrinsèque. De même, la modulation du RRAI, quant à elle, sous la dépendance du système nerveux extrinsèque, n’a que récemment fait l’objet d’une étude spécifique, ayant permis l’adoption de valeurs normatives de modulation, dans une population non neurologique. Nous présentons, ici, une mise au point sur les valeurs normatives de définition et de modulation en amplitude et en durée du RRAI.
Abstract
Anorectal manometry is a useful investigative tool in evaluating bowel symptoms as constipation and faecal incontinence. As the others parameters of anorectal manometry (resting pressure, rectal sensation, pelvic floor and abdominal coordination), no normative values of the rectoanal inhibitory reflex (RAIR) [a physiological modulated reflex involved in anorectal continence, which existence depends on intramural autonomic ganglions and modulation on the integrity of autonomic nervous system] were widely accepted. Nevertheless, normative values of RAIR modulation in terms of amplitude and duration have been recently described. Thus, an existence of altered RAIR modulation can orient to a deregulation of the autonomic system. Here is a clarification of the normal values of RAIR definition and modulation in terms of amplitude and duration.
