Dérivations urinaires continentes (DUC) de type Mitrofanoff ou Yang-Monti chez l’adulte : indications, morbidités, résultats

Objectif

Évaluer rétrospectivement en début d’expérience les indications, les résultats et la morbidité de la réalisation de dérivations urinaires continentes (DUC) de type Mitrofanoff ou Yang-Monti chez l’adulte.

Matériels et méthodes

Quatorze patients ont eu une dérivation urinaire continente: 12 selon la technique de Mitrofanoff (86 %), deux (14 %) selon la technique de Monti. Les indications opératoires consistaient en des difficultés à réaliser des autosondages ou des fistules urétrocutanées secondaires aux autosondages. Quatre patients (29 %) ont eu une iléocystoplastie d’agrandissement associée. En cas d’insuffisance sphinctérienne associée, l’incontinence a été traitée par une fronde aponévrotique sous-cervicale chez deux patients (14 %), une patiente a eu une colposuspension selon Burch, et une patiente a eu des ballonnets péri-urétraux (ACT®).

Résultats

L’âge moyen était de 37,4 ans. Les complications précoces ont consisté en deux occlusions fonctionnelles spontanément résolutives (14 %). Les complications tardives ont consisté en deux (14 %) infections de l’orifice de stomie et en deux sténoses (14 %). Un patient (7 %) a eu une cure d’éventration sur son orifice de stomie. Le suivi moyen a été de 11,2 mois. Tous les patients se sondent sans difficulté hormis une patiente décédée d’une autre cause. Chez les patients avec une hyperactivité détrusorienne rebelle, un traitement par injection de toxine botulinique a été nécessaire dans 21 % des cas (trois patients).

Conclusion

La réalisation de DUC est efficace et peu morbide chez l’adulte. Cela, associé au caractère réversible de l’intervention et la possibilité de traitement endoscopique des complications, rend l’intervention acceptable par le patient.

Objective

To carry out a retrospective assessment of indications, outcomes and morbidity for continent urinary diversion (CUD) based on the Mitrofanoff and Yang-Monti principles in adults.

Materials and methods

Fourteen patients underwent CUD, 12 according to the Mitrofanoff principle (86%) and 2 (14%) using the Yang-Monti procedure. The surgical indications were difficulty performing self-catheterization and urethrocutaneous fistulas secondary to self-catheterization. Four patients (29%) underwent an associated ileal bladder augmentation. In the case of associated sphincter incompetence, incontinence was treated with aponeurotic bladder neck slings in 2 female patients (14%), Burch colposuspension in one patient and periurethral balloons (ACT®) in one other patient.

Results

The average age of subjects was 37.4. Early complications consisted of 2 functional occlusions that spontaneously resolved themselves (14%). Late complications consisted of 2 (14%) infections of the stoma opening and 2 stenoses (14%). One male patient (7%) received an eventration of his stoma opening. The average patient follow-up was 11.2 months. All the patients were able to perform self-catheterization without difficulty, apart from one patient who died because of other reasons. In 21% of the cases (3 patients), there was intractable detrusor overactivity, which had to be treated with an injection of botulinum toxin.

Conclusion

CUD is effective and causes low morbidity in adults. This intervention is reversible, and the possibility of endoscopic treatment of any complications makes it acceptable to patients.
