Zusammenfassung
Durch zahlreiche Neuzulassungen in den vergangenen Jahren hat sich die Therapielandschaft des metastasierten Nierenzellkarzinoms (NZK) signifikant gewandelt. Neben der zielgerichteten Therapie mittels Tyrosinkinaseinhibitoren spielt die Immun-Checkpoint-Inhibition eine bedeutende Rolle in den frühen Therapielinien. Seit September 2019 steht in Europa auch die Kombinationstherapie beider Therapieformen für die Erstlinientherapie zur Verfügung. Die Chance auf ein optimales Ansprechen und eine langanhaltende Remission durch die neuen Therapeutika geht auch weiterhin mit einer hohen Rate an Nebenwirkungen einher. Deren Behandlung erfordert aufgeklärte Patienten und gut abgestimmte interdisziplinäre Behandlungsteams. Der Artikel gibt einen Überblick über aktuelle Behandlungsstrategien für das klarzellige NZK.
Abstract
Due to the approval of multiple drugs for the treatment of metastatic renal cell carcinoma (RCC) in recent years, the treatment landscape has significantly changed. Besides targeted treatment with tyrosine kinase inhibitors (TKI), immune checkpoint inhibition (CPI) plays an important role in the early stages of treatment. The combination of TKI and CPI was approved by the European Medicines Agency (EMA) for first-line treatment in September 2019. The chance for improved response rates and long-term remission using the new drugs is also associated with high rates of side effects. Therefore, treatment with these drugs also requires patients to be well-informed and well-coordinated multidisciplinary healthcare providers. This article provides an overview of current treatment strategies for clear cell RCC.

Für die Identifikation der Schlüsselproteine CTLA‑4 („cytotoxic T‑lymphocyte-associated protein 4“) und PD‑1 („programmed cell death protein 1“) erhielten James Allison und Tasuko Honjo 2018 den Medizin-Nobelpreis. Die Anwendung der gegen CTLA‑4, PD‑1 und PD-L1 („programmed cell death 1 ligand 1“) gerichteten Antikörper sowie die Neuzulassung noch effektiverer Tyrosinkinaseinhibitoren haben die Überlebensdaten und die Ansprechraten von Patienten mit lokal fortgeschrittenem und metastasiertem Nierenzellkarzinom signifikant verbessert.
