Zusammenfassung
Die Cystinose ist eine seltene lysosomale Speichererkrankung, die sich bereits im frühen Kindesalter über ein renales Fanconi-Syndrom manifestiert. Die Cystinose, die unbehandelt einen schweren Krankheitsverlauf nimmt, hat eine Prävalenz von derzeit etwa 200 Erkrankten in Deutschland. Die korrekte Diagnose wird aufgrund der Seltenheit und der zu Beginn unspezifischen Symptome häufig mit großer Verzögerung gestellt. Zu den gesundheitlichen Folgen der unbehandelten Cystinose gehören u. a. terminale Niereninsuffizienz, Erblindung, Hypothyreose, Diabetes mellitus und Rachitis. Bis heute gibt es leider noch keinen kurativen Therapieansatz. Jedoch konnte die cystinentspeichernde Therapie mit Cysteamin deutliche Erfolge bei der Verbesserung der Lebenserwartung und der Lebensqualität der Patienten erzielen. Eine frühzeitige und konsequente Therapie mit Cysteamin kann die Progression der Multiorganerkrankung deutlich vermindern, sodass inzwischen die meisten Patienten erfreulicherweise das Kindesalter überleben und in die Erwachsenenmedizin überführt werden. Die durch Pubertät und durch cysteaminbedingte Nebenwirkungen verursachte Nonadhärenz von Adoleszenten führt häufig zu einer Verschlechterung der Gesundheit im Übergang in die Erwachsenenmedizin. Viele internistische Nephrologen sind aufgrund der Seltenheit der Erkrankung mit dem Krankheitsbild der Cystinose nicht sehr vertraut. Daher sind Schulungen von Patienten, Familien und Ärzten sowie eine neustrukturierte Betreuung von Cystinosepatienten für eine signifikante Verbesserung der Versorgung wünschenswert.
Abstract
Cystinosis is a rare, multisystem lysosomal storage disorder, which is first manifested in early childhood as renal Fanconi syndrome. In Germany, there are approximately 200 patients with this disease and without treatment the disease leads to severe health impairment. Due to the rarity of the disease and the initially unspecific symptoms, a correct diagnosis is often greatly delayed. Without treatment cystinosis often leads to end-stage renal failure, blindness, hypoparathyroidism, diabetes mellitus and rickets. So far no curative therapy is available; however, cystine-depleting therapy with cysteamine has been shown to greatly improve the life expectation and quality of life. An early and consistent cysteamine therapy can reduce the progression of the multiorgan disease, so that most patients now survive childhood and can be transferred into adult care. Consequently, non-adherence often occurring in adolescence and due to the side effects of cysteamine therapy, lead to impaired health of patients with cystinosis during transition to adult medicine. Due to the rarity of the disease many nephrologists are not well acquainted with the symptoms of the disease; therefore, continuing education of patients, families and physicians as well as newly structured, integrated care of patients with cystinosis are desirable for a significant improvement in care.
