Hintergrund
Die Pharmakotherapie ist ein wichtiges Element der Behandlung zahlreicher psychischer Störungen. Auch in der Nephrologie haben Psychopharmaka eine zunehmende Bedeutung. Dialysepatienten entwickeln überdurchschnittlich häufig depressive Symptome.
Ergebnisse
Ungefähr ein Drittel der gängigen Psychopharmaka wird in Abhängigkeit von der Nierenfunktion ausgeschieden. Als Grundregel kann für renal eliminierte Psychopharmaka gelten: Ab CKD („chronic kidney disease“)-Stadium G4 sollte man nicht mehr als die halbmaximale Dosierung geben. Wir haben hier tabellarisch aus unserer NEPharm-Datenbasis die Halbwertszeiten bei normaler und fehlender Nierenfunktion aufgeführt, da die Halbwertszeit auch Auskunft über die Wirkdauer gibt.
Schlussfolgerung
Aus den Werten für die Halbwertzeit können Dosierungsvorschläge abgeleitet werden.
Abstract
Background
Pharmacotherapy has improved the treatment of many psychiatric disorders and psychopharmaceuticals are also gaining in importance in nephrology. Depression has an above average prevalence in dialysis patients.
Results
Approximately one third of all routinely used psychoactive drugs are eliminated by the kidneys dependent on renal function. As a rule drugs eliminated by the renal route should not be given to patients with chronic kidney disease (CKD) stage G4 or higher at more than half the normal maximum dosage. This article presents the half-life values for normal and impaired kidney functions from our NEPharm database in table form because the duration of drug action is correlated to the half-life.
Conclusion
An individualized dose adjustment can be derived from the half-life values.
