Zusammenfassung

Im aktuellen Fallbericht wird ein 24-jähriger Mann beschrieben, der über 4 Jahre bei histologisch gesicherter FSGS mit Cyclosporin A stabil in einer partiellen Remission behandelt wird. Unter Therapie kommt es zum Rezidiv des nephrotischen Syndroms. Zwei neuerlich durchgeführte Nierenbiopsien im Abstand von etwa 1 Jahr zeigen das Bild einer Immunkomplexnephritis in differenter histologischer Erscheinungsform. Als Ursache wird eine Endokarditis lenta diagnostiziert. Nach operativer Sanierung kommt es erneut zur partiellen Remission ohne neuerliche immunmodulatorische Therapie.

Abstract

This report describes the case of a 24-year-old man who was treated with cyclosporin A for histologically proven focal segmental glomerulosclerosis (FSGS) and remained in stable remission. After 4 years the patient suffered a sudden flare of nephrotic syndrome and two renal biopsies at 1 year follow-up intervals showed different forms of immune complex glomerulonephritis. The patient was diagnosed as having endocarditis lenta and was treated surgically. Over the next few months the nephrotic syndrome improved without the need for further immune modulatory therapy.
