Zusammenfassung
Rituximab (RTX), ein chimärer monoklonaler Antikörper, hat erhebliche Bedeutung bei der Behandlung der Vaskulitiden erlangt und ist in den USA seit 2011 für die granulomatöse ANCA-assoziierte Vaskulitis und die mikroskopische Polyangiitis zugelassen. Hier ist der Antikörper bei Erstmanifestation gleichwertig mit Cyclophosphamid, bei Rezidivpatienten sogar überlegen. Die Datenlage ist jedoch noch nicht ausreichend, v. a. in Bezug auf die Remissionserhaltung, sodass RTX zurzeit, auch aufgrund der hohen Kosten, nicht als Mittel der ersten Wahl empfohlen wird. Auch ließ sich keine Reduktion der Nebenwirkungen – und hier v. a. der Infektionsgefährdung – zeigen. Bei der Kryoglobulinämie hat demgegenüber RTX das Cyclophosphamid nahezu verdrängt (siehe Beitrag „Kryoglobulinämische Vaskulitis“ in diesem Heft). Bei den anderen Vaskulitiden ist die Datenlage für eine Empfehlung zu spärlich. Rituximab scheint jedoch bei den therapierefraktären Kleingefäßvaskulitiden zunehmend an Bedeutung zu gewinnen. Zu Polyarteriitis nodosa, Arteriitis temporalis und Takayasu-Arteriitis gibt es nur wenige Fallberichte.
Abstract
Rituximab (RTX), a chimeric monoclonal antibody, has gained importance in the treatment of vasculitis. In 2011 RTX was approved by the US Food and Drug Administration (FDA) for the treatment of granulomatosis with polyangiitis and microscopic polyangiitis as it has been shown to be comparable to cyclophosphamide. In patients with a relapse RTX has shown to give even better results compared to the cytotoxic agent. Nevertheless, as there is insufficient evidence on long-term outcome especially regarding relapse and side effects and taking the treatment costs into account, in Germany RTX cannot be recommended for primary treatment of antineutrophil cytoplasmic antibodies (ANCA)-associated vasculitis. Regarding cryoglobulinemic vasculitis RTX has replaced cyclophosphamide as it has been shown to be very effective. For all other forms of vasculitis there are not enough data to draw any conclusions but RTX seems to be gaining importance for refractory small vessel vasculitis.
